Functional outcome of ligament reconstruction with tendon interposition after failed first carpometacarpal joint prosthesis

The purpose is to determine if ligament reconstruction with tendon interposition (LRTI) a recommendable salvage option for failed total joint prosthesis of the first carpometacarpal joint. Twenty-two patients in our database met in- clusion criteria this retrospective study, at least 6 months follow-up. Fourteen participated and were invited clinical examination asked fill out two questionnaires. They evaluated pain (VAS), impairment (NHS), disability (Quick DASH), opposition (Kapandji test) grip strength (hydraulic dynamometer). Results questionnaires compared cohort study primary LRTI’s. Kapandji test contralateral side. Compared LRTI’s, revision surgery showed mild deterioration disability. average VAS score was 2.9 10. Twelve mentioned sense loss, which could be quantified dynamometer : mean 15.1 kg (operated thumb) versus 20.5 (contralateral). There relatively small decline 8.6 9.9. overall satisfaction good 8 patients, fair 3 poor remaining (mainly based on loss). One patient needed second revision. Failed replacement can salvaged by interposition, providing an acceptable functional outcome 79% cases studied. However, aggravation should taken into account.
